ساختاردهی مسئله تعارضات بین سازمانهای بیمه گر و بیمارستان ها با ترکیب رویکردهای تئوری درام و استراتژی مذاکره و بازتاب مفاهمه

چکیده

اصلاح ساختار نظام سلامت تغییرات زیادی را با هدف بهبود این نظام ایجاد کرده است. از جمله آن می توان به طرح تحول نظام سلامت اشاره کرد که، استقرار مکانیسم های جدید موجب پیچیده تر شدن ارتباطات، بروز چالش ها و در نهایت تعارضات جدی بین سازمانهای بیمه گر و بیمارستانها شده است. این پژوهش، باهدف بهبود چالش های سازمانی بین بیمارستانها و سازمانهای بیمه گر، به بررسی و ساختاردهی تعارضات و تناقضات میان ذی‌نفعان این مسئله پرداخته است.
در این مقاله، ابتدا با استفاده از رویکرد استراتژی مذاکره و بازتاب مفاهمه، مشارکت‌کنندگان مسئله شناسایی شدند. سپس با ترسیم نمودار قدرت-علاقه نقش هر یک از مشارکت‌کنندگان تعیین، و بازیگران اصلی شناسایی شدند. در ادامه با استفاده از تئوری درام، مسئله تقابل بازیگران در این مسئله ساختاردهی گردید. بدین منظور اولویت‌ها و گزینه‌های تصمیم هر یک از بازیگران در یک میز مذاکره، مورد تحلیل قرارگرفته و معضلات عدم همکاری و کشمکش بین آنها ارزیابی شد. درنهایت راهکارهایی برای حل معضلات در این مسئله ارائه گردید.
نتایج مطالعه نشان داد که بیشترین کشمکش میان سازمانهای بیمه گر و بیمارستان ها شکل گرفته و مهمترین نوع معضل بین سازمانها، معضل رد گزینه های یکدیگر است.
تعارضات ناشی از معضل رد گزینه ها را می توان بهترین نوع از تعارض دانست که با مذاکره اندکی انعطاف پذیری و سازش در مورد گزینه های موجود و یا با اندکی تغییر و نوآوری در ارائه گزینه های جدیدتر، می توان این تعارضات را به همکاری تبدیل نمود.

چکیده [English]

The reform of the structure in the health system has made many changes with the aim of improving this system. One of them is the health system transformation plan, which has led to the introduction of new mechanisms, which have complicated communication, challenges and, finally, serious conflicts between insurance organizations and hospitals.In this study, in order to improve the inter-organizational problems, the conflicts and contradictions between the stakeholders of this issue have been studied and structured.
In this paper, participants were first identified using the journey making. Then, by drawing a power-interest diagram, the role of each participant was determined, and the main actors were identified. Next, the problem of hospital waste management actors was structured by drama theory. Therefore, the priorities and decision options of each of the actors at a negotiating table were analyzed and the problems of non-cooperation and conflict between them were evaluated. Finally, strategies to solve problems in the management of hospital wastes were presented.
The results show that the most conflict between waste organizations and hospitals is formed and the most important type of problem between organizations is the problem of rejecting each other's options.
Conflicts caused by rejection dilemma can be considered as the best type of conflict that can be turned into cooperation by negotiating options and a little flexibility, compromise and, of course, innovation in offering newer options.
